The North Country Health Consortium (NCHC) is an independent, non-profit, public health organization and rural health network dedicated to improving the health status of Northern New Hampshire residents for more than 20 years. The NCHC service area is composed of Coös County and Northern Grafton County, located in the beautiful White Mountains.  People in New Hampshire call this region the North Country. Like many rural areas, the North Country is underserved, but the quality of life here is among the best.

NCHC’s initiatives focus on the creation and sustainability of a cohesive regional health care delivery network. NCHC’s systems improve the infrastructure capacity and delivery of public health services; improves access to services for underserved and uninsured North Country residents; offer training and development to attract and retain qualified health professionals; and provide direct care services related to substance use disorder and chronic disease.  


North Country Health Consortium seeks a Community Health Worker (CHW) Coalition Outreach Coordinator to join the Ways2Wellness CONNECT team. W2W CONNECT deploys trained CHWs to work with individuals who are living with the challenges of chronic disease(s) to meet their goals. The CHW Coalition Outreach Coordinator will provide local/regional and statewide outreach, support, and coordination to promote W2W CHW services, enhance program implementation, as well as drive NH CHW Coalition engagement to evolve efforts of the Coalition. This full-time opportunity (40 hrs.) is partially remote and reports to the CHW Program Manager.  

Requirements:
  • Graduation from an accredited college or university with a Bachelors Degree in Public Health, Nursing, Social Work, or other health related field; or any equivalent combination of experience and training which provides the knowledge, skills and abilities necessary to perform the work
  • Experience with community organizing; vast knowledge of the community; and working with partners from all sectors
  • Strong written, oral, and relationship building skills
  • At least 2+ years of residency within the local community, with knowledge of local culture and values, and familiarity with the resources available within the community
  • Willingness to travel within NH
  • Valid driver’s license, reliable transportation, and proof of insurance
  • Willingness to complete CHW training (provided by NCHC)
Responsibilities:

● Conduct needs assessment to identify professional development needs of CHWs in NH ● Develop plans for CHW ECHO series in conjunction with program management

● Develop and implement a CHW awareness program for racial/ethnic minorities and rural populations to promote recruitment for CHW training and increased workforce capacity● Work collaboratively with partners to coordinate training, meeting, and education schedules ● Work with management to evaluate the impact of CHW Coalition activities relating to COVID-19 prevention and management and other Social Determinants of Health ● Provide support and assistance to evolve the efforts of the NH CHW Coalition to further certification and statewide integration of the CHW workforce
