Community Case Managers

Lakes Region Mental Health Center

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.

    Community Case Manager - Community Support Program
    Targeted Case Manager - Community Support Program
    Community Case Manager - Assertive Community Treatment (ACT) Team
    Community Case Manager - Substance Use Disorder Team
    Community Case Manager - Neurocognitive Team

 

This is a great opportunity to apply your clinical skills while treating patients with persistent mental illnesses as a part of a multidisciplinary team.

Case Managers are responsible for coordinating and assuring the continuity of services for each patient on their caseload.  This includes working with each patient and caregiver(s) to coordinate care planning, complete a comprehensive assessment of needs, develop a Case Management Plan, monitor the effectiveness of it, plus ensure resource linkage, referral, and advocacy.  May also coordinate care and maintain the medical record within the agency.

Requirements:

Bachelor’s degree in Social Work, Psychology, Human Services or other related field preferred but not required. One - three years of experience in mental health preferred, or lived experience. Valid driver's license, insurance and vehicle required.
