Community Health Worker Education Coordinator

The North Country Health Consortium (NCHC) is an independent, non-profit, public health organization and rural health network dedicated to improving the health status of Northern New Hampshire residents for more than 20 years. The NCHC service area is composed of Coös County and Northern Grafton County, located in the beautiful White Mountains.  People in New Hampshire call this region the North Country. Like many rural areas, the North Country is underserved, but the quality of life here is among the best.

NCHC’s initiatives focus on the creation and sustainability of a cohesive regional health care delivery network. NCHC’s systems improve the infrastructure capacity and delivery of public health services; improves access to services for underserved and uninsured North Country residents; offer training and development to attract and retain qualified health professionals; and provide direct care services related to substance use disorder and chronic disease.  


North Country Health Consortium seeks a Community Health Worker (CHW) Education Coordinator to join the Ways2Wellness CONNECT team. W2W CONNECT deploys trained CHWs to work with individuals who are living with the challenges of chronic disease(s) to meet their goals. Activities of the CHW Education Coordinator will be centered on managing the logistical components of the Community Health Worker Training and providing outreach, marketing coordination, and promotion for the training to increase CHW workforce capacityThis position is a part-time opportunity (20 hrs.) and reports to the CHW Program Manager.

Requirements:

QUALIFICATIONS/REQUIREMENTS

  • Graduation from an accredited college or university with a Bachelors Degree in Public Health, Nursing, Social Work, or other health related field; or any equivalent combination of experience and training which provides the knowledge, skills and abilities necessary to perform the work
  • Strong written, oral, and relationship building skills
  • At least 2+ years of residency within the local community, with knowledge of local culture and values, and familiarity with the resources available within the community
  • Willingness to travel within NH
  • Valid driver’s license, reliable transportation, and proof of insurance
  • Willingness to complete CHW training (provided by NCHC)
Responsibilities:

JOB DUTIES 

Manage CHW training logistics, including scheduling, registration, implementation, and evaluation of trainings Build a cadre of CHW training faculty to support peer to peer training 
Coordinate specific training for community-based CHWs on connecting patients with community health and wellness partners regarding COVID-19, chronic disease prevention, management and screening  Work with CHW Outreach Coordinator to conduct outreach and marketing for all CHW training components to promote recruitment for training and increase workforce capacity Participate in the needs assessment to identify professional development needs of CHWs in NH Maintain tracking and reporting processes for program activities
