The Intake Coordinator is responsible for scheduling and conducting initial program assessments; conducting daily callbacks and providing referral information for those seeking services; collaborating with area service providers; gathering documentation for program eligibility; data entry; managing incoming files and initiating move-in process. The intake coordinator also supports groups and potentially a small individual caseload. 

 *Families in Transition is mandating the COVID-19 vaccination upon employment. You must provide proof of vaccination prior to the first day of employment*

Requirements:
  • Flexible work schedule.

  • Maintain a professional appearance at all times. 

  • Criminal Background Check

  • Valid Driver’s License

  • Must have and maintain LADC, MLADC, LICSW, MLADC or combination in state of NH or within six months of obtaining licensure

  • High school diploma/equivalent

  • Pre-Employment Physical / 2 Step TB Test

Responsibilities:
  • Complete all initial intakes, via telephone, walk-in or e-mail, providing quality support, crisis management, referrals and agency information to each individual intake. Maintain intake schedule on an on-going basis.

  • Knowledge and practice with the DSM5 criteria for substance use and mental health disorders 

  • Participate in an integrated health care model, working with participant’s primary health care providers, state agencies, and other collateral contacts within participants’ care. Provide care coordination and case management services based on client’s needs

  • Facilitate about one evidence based groups weekly and support a small caseload of approximately 8 clients. 

  • Knowledge of mandating reporting and ability to call appropriate agencies with safety concerns. 

  • Ability to work on a team, collaborate with other professionals internally and externally, participate in effective interpersonal communication on a daily basis.

  • Knowledge of and Abide by the requirements of 42 CFR and HIPPA requirements

  • Maintain records and storage of closed participant files.

  • Practice cultural competency with establishing rapport and relationships with participants from other cultural and ethnic backgrounds  

  • Participate in weekly team meetings and ongoing clinical supervision

  • Preform observed drug testing as appropriate

  • Complete and submit reports as requested.

  • Participate in quality improvement practices  

  • Demonstrate a positive, enthusiastic and motivating presence at all times. 

  • Participate in in-service training. 

  • Maintain and promote professionalism, in accordance with the Families In Transition Employee handbook

  • Perform other duties assigned by the Program Manager

$48,000 - $51,000
