Team Coordinator, Adult Services

Job Summary:

Greater Nashua Mental Health is looking for a full-time Team Coordinator to join our Adult Services Team! The Team Coordinator clinically and administratively manage teams within the Community Support Services Department.  The Team Coordinator is responsible for the Team’s performance on KPI’s, including productivity expectations and paperwork completion.  The Team Coordinator oversees the professional development of direct reports.  The Team Coordinator expands Evidence Based Clinical Practices within the department, spearheads and champions quality initiatives, and refines clinical practice guidelines, processes, and procedures within the department.


Duties & Responsibilities:

  • Provides weekly one (1) hour individual supervision to master's level therapists (four (4) to six (6) individuals) and senior case manager.  Provides biweekly one (1) hour supervision to case managers (eight (8) individuals). Completes all documentation of the supervision sessions in a timely manner.
  • Provides ad hoc support to staff for crisis management and guidance.  Also provides supplemental counseling to members of clients’ support systems (e.g., family members).
  • Provides a minimum of ten (10) hours per week of psychotherapy for individuals, groups, and/or families, as well as case management services.
  • Completes treatment plans, Adult Needs and Strength Assessments (ANSA) and Case Management Care Plans (CMCP) by the required due date listed in EMR and supports and monitors staff doing the same.
  • Completes all Quarterly Reviews within a two (2) week window at the end of each review period, and completes all other documentation within three (3) business days (with the first day being the dates of service/completion) and supports and monitors staff doing the same.
  • Prepares for and attends team meetings (2 per week), therapists' meeting (1 per week), department meeting (1 per month), and supervision (1 per week).
  • Performs other duties or special projects as required or as assigned.



  • Master’s Degree in Clinical Mental Health Counseling or Social Work.
  • Must be licensed or license-eligible in the State of NH (LICSW, LCMHC).
  • 3-5 years of related work experience
  • Ability to teach effectively.
  • Familiarity with community resources.
  • Strong collaborating skills and an ability to work effectively both independently and as part of a team.
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
