Psychiatric Mental Health Nurse Practitioner

Job Summary:
Greater Nashua Mental Health is looking for a full-time Psychiatric Mental Health Nurse Practitioner (APRN) to provide direct psychiatric evaluation, management, and psychiatric supervision to GNMH clients and pediatric clients referred from St. Joseph Hospital’s 3 pediatric clinics located in Nashua and Milford, NH. This position will have an opportunity for both remote and in-person work.

The Psychiatric Mental Health Nurse Practitioner provides psychiatric or mental health input and guidance to the team's psychiatric or mental health management of clients during team meetings, reviews the mental health treatment plan and ensures that it identifies appropriate problems, goals, and objectives, and provides psychoeducation to patient and family regarding diagnosis, medications, and treatment plans.  The Psychiatric Mental Health Nurse Practitioner will manage medications until it is appropriate for the patient to be referred back to their PCP for ongoing care, or refer the patient to GNMH for specialty level of care.

Duties & Responsibilities:
• Provides direct patient care by performing psychiatric evaluations and medication management follow up visits.  Reviews records and collateral information in preparation of the visits, and ensures that 95% of PHQ-9 responses are addressed.  Addresses measures of metabolic syndrome at 90% of visits.
• Collaborates with Integrated Care Therapist, PCP, and nursing staff of St. Joseph Hospital’s pediatric clinics.
• Complies with the policies and protocols that apply to medical staff of both St. Joseph Hospital’s pediatrics clinics and GNMH.
• Participates in relevant team meetings and provider meetings.
• Promotes the quality of patient care through participation in weekly, and as-needed supervision with the GNMH CMO, and maintenance of nursing/medical/psychiatric education.

Qualifications:
• Master’s Degree, Psychiatric Mental Health Nurse Practitioner.
• NH accredited Nurse Practitioner License.
• 1-2 years of related experience.
• Knowledge of psychiatric diagnoses, medications, and treatments prescribed to clients.
• Experience using electronic medical records (EMR) systems.
• Strong collaborating skills, and an ability to work effectively both independently and as part of a team.
• Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
• Excellent communication and interpersonal skills.
• Ability to multitask, and be flexible with regard to workload and assignments.
• Ability to exercise good judgment and sound decision-making.
