Older Adults and Neurocognitive Community Case Manager

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.


Case Managers are responsible for coordinating and assuring the continuity of services for each patient on their caseload.  This includes working with each patient and caregiver(s) to coordinate care planning, complete a comprehensive assessment of needs, develop a care plan, and monitor the effectiveness of it, plus ensure resource linkage, referral, and advocacy.  Also coordinate care and maintain the medical record within the agency.  Provide services for Older Adult and Neurocognitive Team Patients that reside within the Laconia Housing Authority residences.  Provide Referral, Education, Assistance and Prevention (REAP) assistance to individuals and in group settings.

Requirements:

The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how the individual can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities.  Must also poses the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.
1.Working knowledge of diagnosis, treatment planning, medications, counseling and crisis intervention techniques.
2.Must be able to establish a rapport with the patients.  Must treat all patients with dignity and respect.
3.Ability to organize work-load so that his/her clinical and administrative duties are completed in a timely and thorough manner without the need for direct supervision.
4.Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts.
5.Understands the process for providing REAP services
6.Must be able to obtain certification to administer the MoCa through provided training
7.Attends monthly meeting with REAP counselors.
8.Must be capable of working closely with other agency staff to expand the knowledge of mental health services both within the Child and Family Team and across to other programs. 
9.Must be able to develop strong working relationships with external services.
10.Must have basic computer skills, and strong verbal and written communication skills.
11.Must have the ability to read and analyze data and reports.
12.Must be able to tolerate periods of heavy workload, to include long working hours and the balancing of conflicting demands
 
Qualifications:

Education:  Bachelor’s degree in Social Work, Psychology, Counseling or other approved credentials as specified in He-M426-11 and He-M426-13.
Experience:  1 to 2 years’ experience preferred.
License/Certifications:  Driver’s license.  Able to pass LRMHC criminal records and motor vehicle screening.   Must maintain personal auto insurance at a level commensurate with agency requirements.
 
Work Schedule:
Generally, works Monday through Friday with occasional requirements to cover emergencies after hours.

Responsibilities:

Under the direction of the Older Adults and Neurocognitive Program Manager, and within the center’s program policies and procedures, performs the following functions:

Essential Functions:
1.Meet with patients/caregiver(s) (face to face) at intake, periodically and annually to obtain information to conduct a comprehensive assessment of needs.
2.Ongoing communication and coordination with patient/caregiver(s), and providers/collaterals, as needed.
3.During periodic face to face meetings revise Case Management Plan to document any newly identified needs and /or services.
4.Review and monitor linkage to services identified in the Case Management Plan, link patients to services, including but not limited to: phone calls, scheduling appointments, verbal and written referrals to internal/external providers and the completion of associated documents.
5.Promote and ensure a person-centered planning approach to care and patient choice of services.
6.Meet quarterly with patient and internal providers to obtain information to complete Quarterly Service Review documentation, including review of records, summaries and assessments from current providers.
7.Complete all documentation and billing in compliance with standards set by state and agency policy.
8.Attend clinical meetings to discuss patients as needed.
9.Provide REAP Program services as assigned by Program Manager.
10.Provide REAP education sessions within Laconia Housing sites.
11.Complete all assessments and documentation as required by REAP
12.Attend meetings, trainings, and in-services as required and participate in regularly scheduled supervisions as required.
13.Attendance is an essential function of this position.
