Manager of PCA Program

The Manager is responsible to provide mentoring to the RN Coordinators statewide and work in collaboration with the Sr. Director to implement a unique hybrid model of community health care services (PCA, NF Tran, IL Services). Specific responsibilities include but not limited to provide RN Service Coordination, assure compliance with state/federal rules and best practices; also assist/provide marketing and outreach to the community.

Responsibilities

Service Coordination:

· Participate in the assessment and evaluation process for determination of eligibility for participants in PCA program.

· As needed, assist with recruitment, hiring, and training of new ACEs with assigned consumers.

· Teach and provide skills training to consumers on how to run a consumer directed program.

· Review consumer’s care plans to ensure attendant care needs are being met and adjust as needed. Assure all paperwork is completed in accordance with GSIL policy & procedures

· Consult with individuals about health care concerns and provide information on available options.

· Provide orientation and ongoing skills training to consumers regarding attendant care program policies and procedures. Address health care management issues and medical planning, assist with development of a back-up plan.

· Make face to face visits to monitor and review participant’s care plan at a minimum quarterly and more often as the consumer’s needs change, including 6 month and annual reassessments.

· Coordinate services and maintain accurate records for consumers. Assure consumer records are maintained in accordance with GSIL policy and procedures.

· Be back up for the Sr. Director/Director in their absence.

· Assist consumers with Nursing Facility Transitions and monitor status on an ongoing basis.

· Supervisions of department RN’s and attendant care specialist.

 

Mentoring:

· Provide orientation and skills training to RN’s/Staff and consumers regarding attendant care program policies and procedures. Address health care management issues and emergency medical planning. Promote health and wellness initiatives that will provide information for consumers and training for staff in supporting individual’s wellbeing.

· Act as a support, with the coordinators, to work collaboratively to resolve and best support consumer specific situations for community living.

· Assist in developing consumer specific strategies to best support consumers on how to self-direct services and manage their workers.

· Provide Back up support/coverage in the absence of an assigned RN coordinator

· Develop and conduct presentation and teaching of the department trainings, in collaboration with the Sr. Director.

· Participate in Attendant Care Education trainings on a rotation basis for all new attendant care employees.

 

Best Practices/Regulatory compliance:

· Monitor and assist the Sr. Director to assure regulatory compliance through mentoring RN coordinators.

· Assist the Sr. Director to develop ‘best practices’ of support for consumer directed services that advance preventive and responsive health care and self-direction.

· Seek opportunities to design and implement, in collaboration with Sr. Director new and innovative community/health care service models.

 

Outreach and Community Education:

· Serve as an information resource for community and staff on personal care options, community supports, and health and disability care related issues.

· Provide outreach to provide training and education to community organizations and Managed Care Organizations, in collaboration with Director.

· Assist with outreach efforts to increase awareness of GSIL programs.

 

Required Qualifications

· Registered Nurse with current NH License.

· 2+ years working with individuals with long term health care needs.

· Effective Communication and teaching skills.

· 2+ years supervisory experience.

· Commitment to facilitating and maximizing independence for person with disabilities.

· Experience with person centered philosophy.

· Work a flexible work schedule in accordance with department needs.

· As needed, travel to regional office throughout the state.

· In the absence of the Sr. Director be point of contact for internal and external partners.
