We offer many programs specifically focused on our Military Veteran and homeless populations.  Harbor Care integrates stable housing with vital supports such as: primary, dental, and mental health care, substance misuse treatment, employment services, and other wraparound supports proven to end homelessness, help change and save lives.


Department:              Primary Care/Harbor Care Health & Wellness Center

Reports to:                 Director of Nursing

Pay Status:                  Full Time Non-Exempt

Schedule:                     Monday through Friday; 8:00am – 5:00pm.

Will be required to work occasional evenings and Saturdays.

 About the Harbor Care Health and Wellness Center:

Harbor Care Health and Wellness Center offers primary, preventive medical services, behavioral health services, and dental services to children and adults who are homeless or at risk of homelessness, uninsured, under-insured, or low income.   Harbor Care offers a sliding scale fee for self-pay patients, and accepts Medicaid, Medicare, as well as most commercial insurances. The Southern NH HIV/AIDS Task Force has a long history of providing culturally sensitive support services to low-income individuals living with HIV/AIDS. With medical case management at the heart of our program, each client is assisted in creating a service plan with action steps leading to viral suppression while meeting their other basic needs.

 

Job Summary:

The Charge Nurse is responsible for managing the clinical support staff and operations provided within the Harbor Care Health & Wellness Center (HCHWC). The Charge Nurse is a professional employee, trained to provide health related services.  The Charge Nurse functions under the direction, instruction and supervision of the Director of Nursing. This includes a combination of administrative and medical practice responsibilities, which are related to the supervision of clinical support staff, policies, procedures, and practice. The Charge Nurse is actively involved in the coordination of patient care in order to ensure the smooth progression during the patient’s appointment and follow-up. 

Requirements:

Basic Qualifications:

  • Bachelor’s Degree in Nursing
  • Current registration and licensure in the State of New Hampshire
  • Two (2) years of Medical/Surgical experience
  • 4 years’ experience as an Outpatient Clinic Charge Nurse
  • Proficient MS Office
  • Valid CPR Certification

  

Preferred Qualifications:

  • Knowledge of computers and software applications or ability to learn quickly.
  • Experience working with people from diverse populations
  • Ability to work as an effective team member is essential.

  

Knowledge, Skills and Abilities:

  • Be aware of and adhere to all HIPPA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients/employees and this Agency.
  • Excellent time management skills.
  • Ability to listen, communicate (written and verbal), excellent grammar, spelling and proofreading skills and follow-up effectively with all staffing levels and clients/customers.
  • Ability to work independently, self-starter and energetic.
  • Ability to function in a team-oriented environment.
  • Ability to handle highly confidential information.
  • Attention to detail.
  • Able to adhere to tight deadlines.

  

Essential Duties and Responsibilities:

The assigned duties and responsibilities include but are not limited to:

  • Complete all necessary paperwork related to the position in a timely manner
  • Oversees orientation of new HCHWC staff
  • Coordinates and facilitates Clinical Support Staff meetings
  • Supervises Medical Assistants.
  • Insures effective communication and coordination among team members through training and effective team meetings
  • Works with management team to ensure nurses and medical assistants are working at the top of their license / certificate. 
  • Works with management team to establish and implement clinical policies and procedures.
  • Responsible for performance of clinical support staff.
  • Evaluates information against standards while continuously monitor and control resources.
  • Develop and build teams through guidance and professional coaching efforts ensuring that teams are knowledgeable regarding patient population management and effective communicators for all segments of the patient population, particularly vulnerable populations.
  • Organize, plan, and prioritize work in accordance to HCHWC and HHI performance measures and objectives.  
  • Ensures HCHWC complies with medical standards, regulations, and laws created by regulators, accrediting organizations, and state or federal government agencies
  • Participates in HCHWC Quality Assurance/Quality Improvement efforts.
  • Liaison to the following operations of the HCHWC: Patient billing and insurance, purchasing, facilities management, reception, forms, data collection and reports, and IT as they relate to clinical services.
  • Provides anticipatory guidance, screening and health education to patients to promote and maintain health
  • Conduct oneself in a manner that promotes safety
  • Other duties as required by Clinic Director.
