Elders Outreach Counselor

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a part-time Elder’s Outreach Counselor to join our talented multi-disciplinary team in our Community Support Program.

As an Elder’s Outreach Counselor, you will work with REAP, a statewide, community-based outreach program providing Referral, Education, Assistance and Prevention services to older adults (60 years old or older) who live in NH, their family members and caregivers, and other professionals who work for and on behalf of older adults; and individuals who live in senior housing sites and who are younger than age 60.

At Riverbend, we have implemented processes and precautions to help prevent the spread of COVID-19 as well as a mandatory vaccination policy.  Exceptions can be applied for under medical or religious reasons. Additionally, weekly testing will be required for anyone who is in progress of the vaccine series or individuals with approved exceptions.

About working at Riverbend 

This is a part-time, 20 hours per week position with a prorated benefit package including:

  • 403(b) retirement plan with company match
  • 4 weeks of vacation time accrued in first year, higher accrual in future years
  • 8 holidays
Requirements:
  • Bachelor’s Degree in Psychology, Social Work, Education, Rehabilitation or Human Services is required.
  • Experience and interest in working with older adults and a willingness to learn how to integrate prevention information into practice is preferred.
  • A valid driver’s license, personal vehicle, and auto insurance is required.
Responsibilities:
  • Participate in initial and annual REAP training
  • Understand community resources available to older adults, including benefits, housing, medical care, etc
  • Collaborate with individual's support network and other service providers
  • Review referrals and provide community-based REAP services
  • Provide education sessions to individuals who work with older adults
  • Participate in monthly REAP meetings with the state REAP supervisor
