Community Action Partnership of Strafford County is a 501(c)(3) private nonprofit organization established in 1965 under the provisions of the Equal Opportunity Act of 1964. We work with community, state and federal partners to assist children, families, low-income and elderly residents with their efforts to become or remain financially and socially independent through a variety of coordinated programs. Without the services provided by our agency, many local residents would be without a means to provide for their basic needs, including food, education, child care, utilities assistance, transportation, shelter and access to other services.

General Description

Responsible for supporting staff and ensuring high-quality programming for home visiting programs. Ensures compliance with Healthy Families America (HFA) Best Practice Standards and all State and Federal regulations.

Education/Experience

  • Minimum of a bachelor's degree in Human Services or related field.
  • At least three (3) years of related experience in service delivery to culturally diverse families with young children using a strengths-based approach, including at least two (2) years supervisory experience preferred.
  • Knowledge of community resources in Strafford County.
  • Advanced computer literacy including Excel, Word, and Outlook.

Duties and Responsibilities 

· Works with the Home Visiting Manager and Outreach and Enrollment Coordinator to ensure full enrollment.

· Provides weekly supervision and support to HFA home visiting staff in Early Childhood Development, Prenatal Services, Family Support, Nutrition and Parenting in accordance with HFA program requirements.

· Participates in the orientation of all new HFA home visiting staff. Directly facilitates the professional growth of the home visitor.

· Maintains accurate and up-to date information about program policies and procedures, shares with HFA Home Visiting staff and ensures adherence to such policies and procedures.

· Ensures that all written and electronic records are complete and up to date.

· Regularly observes and documents the work of the HFA home visitors; monitors and maintains caseloads.

· Reviews HFA Screens and determines the need for completion of Parent Survey.

· Accepts for enrollment those determined to have the highest needs.

· Maintains a limited caseload of families under HFA and/or Comprehensive Family Support Services (CFSS) home visiting programs providing resources and referrals and/or home visiting services to families in need.

· Facilitates ongoing parent groups with relevant topics for parent education which will promote socialization, child development, and/or recruitment of other families.

· Completes semi-annual audit of participant files to ensure all records are up-to-date, complete and accurate.

· Sees that all necessary administrative tasks are completed in a timely manner including required reports and supervision records.

· Meets weekly with Home Visiting Manager for reflective supervision if has a caseload or monthly if no caseload.

· Attends HFA/CFSS state meetings and trainings.

· Facilitates timely, appropriate reports of suspected cases of child abuse and neglect to DCYF.

· Assists in the development of and monitors work plans to achieve targeted performance measures for HFA.

· Ensures program adherence to all relevant HFA Home Visiting program requirements, recommending changes to procedures and updating forms.

· Attends all meetings and/or conferences as assigned (community, state, regional, and national).

· Completes Medicaid billing for CFSS, as applicable.

· Other duties as may be assigned.
