Elders Outreach Counselor

Core services include 24/7 emergency mobile crisis services; counseling services for children, adults, and families; addiction treatment; and community support, vocational services, and residential programs for adults and seniors. Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a part-time Elder’s Outreach Counselor to join our talented multi-disciplinary team in our Community Support Program.

As an Elder’s Outreach Counselor, you will work with REAP, a statewide, community based outreach program providing Referral, Education, Assistance and Prevention services to older adults (60 years old or older) who live in NH, their family members and caregivers, professionals who work for and on behalf of older adults; and individuals who live in senior housing sites and who are younger than age 60.

  • Bachelor’s Degree in Psychology, Social Work, Education, Rehabilitation or Human Services is required.
  • Experience and interest in working with older adults and a willingness to learn how to integrate prevention information into practice is preferred.
  • A valid driver’s license, personal vehicle, and auto insurance is required.


  • Participate in initial and annual REAP training
  • Understand community resources available to older adults, including: benefits, housing, medical care, etc
  • Collaborate with collaterals and other service providers
  • Review referrals and provide community based REAP services
  • Provide education sessions to individuals who work with older adults
  • Participate in monthly REAP calls with the state REAP supervisor
