Community Support Network, Inc. (CSNI) is seeking a motivated and self-starting individual to join our team of Supports Intensity Scale (SIS) interviewers. The primary location for these interviews will be held in the Monadnock region in NH. The SIS Interview assists individuals with developmental disabilities and acquired brain disorders and their team with identifying the supports they need to live a satisfying life. SIS interviews are conducted using a nationally-recognized tool, and include the individual being served as well as those in the individual’s life who provide supports, including family members and paid supports. SIS interviewers work remotely and travel to various regions of the state in order to conduct interviews. Reliable transportation and willingness to travel are a must for this position. Position Summary:

Responsible for conducting Supports Intensity Scale (SIS) interviews and data entry of all information collected.

Essential Duties:

  • Possess a solid understanding of the Supports Intensity Scale (SIS) process and the tool, training will be provided.
  • Represent CSNI, BDS and NH Area Agencies as an ambassador of the SIS Program. Present a positive view of the process.
  • Conduct SIS interviews with individuals statewide; collect information and data that informs eligibility decisions.
  • Accurately enter all interview data into the SIS online database in a timely manner, as required, and proof for accuracy.
  • Undergo observation and rating of 10% of interviews annually by another certified NH or AAIDD Trainer to ensure consistency and NH and AAIDD interview standards.
  • Attend regular SIS team meetings for the purpose of interviewer support, operational issues/updates, sharing experiences, new approaches, recommended changes, problem solving for proving the overall effectiveness of the SIS process.
  • Participate in AAIDD or CSNI trainings related to the SIS process or skills required. 
  • Maintain current AAIDD certification.
  • Other duties as requested or needed.

Experience, Skills and Educational Requirements:

  • Minimum of 5 years’ experience working with people with an intellectual/developmental disability or an acquired brain disorder. Knowledge of standardized educational and vocational evaluations a plus.
  • Successful completion of national AAIDD training.
  • Experience with Microsoft Office Word, Excel and other software products.
  • Organized, able to multitask, meet deadlines and deliver high quality results.
  • Excellent verbal and written communication skills.
  • Excellent customer service skills and interpersonal style.
  • Positive, professional attitude and overall demeanor.
  • Availability to work a varied schedule and to work when needed.
  • Ability to travel regularly to various locations; must possess a valid NH license and a working vehicle with proof of insurance coverage.
