Emergency Services Community Case Manager

Lakes Region Mental Health Center

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.


Assists patients to discover and maintain a valued role(s) in the community, brokers services and service links with outside agencies and interfaces within the agency to ensure optimum quality and comprehensiveness of services

Requirements:

The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how they can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities.  Must also posses the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

1.Working knowledge of diagnosis, treatment planning, medications, counseling and crisis intervention techniques.
2.Must be able to establish a rapport with the patients.  Must treat all patients with dignity and respect.
3.Ability to organize work-load so that his/her clinical and administrative duties are completed in a timely and thorough manner without the need for direct supervision.
4.Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts.
5.Must be capable of working closely with other agency staff to expand the knowledge of mental health services both within Emergency Services and across to other programs.  Must be able to develop strong working relationships with external services.
6.Must have basic computer skills, and strong verbal and written communication skills.
7.Must have the ability to read and analyze data and reports.
8.Must be able to tolerate periods of heavy workload, to include long working hours and the balancing of conflicting demands

Qualifications:

Education:  Bachelor’s degree in Social Work, Psychology, Counseling or other approved credentials as specified in He-M426-11 and He-M426-13.  May substitute lived experience of mental illness for degree.

Experience:  1 to 2 years’ experience preferred.

License/Certifications:  Valid NH driver’s license.  Able to pass LRMHC criminal records and motor vehicle screening.   Must maintain personal auto insurance at a level commensurate with agency requirements.

Work Schedule:

Generally, works Monday through Friday with occasional requirements to cover emergencies after hours.

Responsibilities:

1.Monitors patients with Presumptive Eligibility, creates a Case Management Plan for these individuals and performs targeted case management services by assisting patients in gaining access to needed benefits (to include presumptive eligibility for Medicaid), and medical, social, educational and other services.
2.Determines clinically appropriate routine, urgent or emergent services for ES calls and/or walk-ins
3.In collaboration with the Emergency Services (ES)Clinician and/ or Stabilization Clinician, initiates intakes for eligible clients and assists ES Clinicians in other documentation tasks such as prior authorizations
4.Ensures continuity of care, effective discharge planning and care coordination for clients seen by ES and/or stabilization staff and may work with Hospital Liaison in this regard
5.Provides crisis intervention to individuals, presenting with emergent and/or urgent needs when ES is not immediately available
6.Maintains a database that contains eligibility, intake and disposition information
7.Completes required documentation in a timely and thorough manner, according to established standards
8.Participates actively in supervision, assigned meetings, and team meetings; makes contributions and applies what is learned effectively
9.Manages a variety of tasks and demands.  Prioritizes competing needs in a way that is most helpful to patients and is an efficient use of time
10.Participates in team activities and offers support for patient/staff/program/agency initiatives
11.Attendance is a requirement of this position
