Collaborative Care Clinician (Northeast Rehab Hospital sites)

The Collaborative Care Clinician provides behavioral health assessment and consultation to adults as part of an innovative integrated health care model established at 4 different medical locations in Southern NH.   Working collaboratively with physicians, nurses, and community providers, the Collaborative Care Clinician strives to integrate treatment utilizing a systems’ framework that supports a continuum of care model in pursuit of improved outcomes for clients/patients. 

This position is currently part-time/fee for service.  Work schedule would consist of a 4-hour time block at a specific location working up to 16 hours per week.  Flexible times for work hours will be considered.


Qualifications and Skills

  • LICSW required;
  • Experience with integrated health care highly preferred;
  • Demonstrated ability to communicate effectively with professionals and clients/patients;
  • Must have advanced clinical assessment and documentation skills;  
  • Must have excellent organizational skills and the ability to be efficient in a fast pace setting.

Physical Demands and Working Conditions

Must have the ability to sit at computer and use a keyboard for completing clinical notes as well as sit through therapy sessions; must be able to flex work schedule to accommodate needs of patients and staff; must have reliable transportation to travel to offices.


Major Functional Areas and Description of Duties

  • Provides behavioral health assessments in varied health care settings.
  • Provides consultation and recommendations as part of an integrated health care team.
  • Utilizes solution-focused strategies and treatment that is outcomes driven.
  • Communicates effectively and collaboratively with members of the medical teams and embraces a “warm hand-off” protocol. 
  • Provides consultation/education to provider teams on topics related to behavioral health, individuals with special needs, and continuum of care.
  • Functions professionally and in a manner that protects the integrity, confidentiality and rights of all clients/patients.
  • Provides clinical documentation that is completed on a timely basis and in compliance with appropriate CLM, federal, state and other pertinent standards.
  • Maintains professional memberships, licenses, accreditation, certifications, etc.
  • Works in conjunction with the program director to insure that service delivery is efficient and effective.
  • Performs all other duties as assigned by supervisor.
