We’re offering a $500 Sign-On Bonus* for Family Case Managers!

We have an exciting opening for a Family Case Manager to work with families to provide in-home supports for their school-age children with developmental disabilities, such as autism or Down syndrome. It’s a great opportunity to make a difference in the lives of children and their families by ensuring they have access to the services they need to thrive and grow.

You’ll work closely with families to evaluate their needs and determine appropriate and cost-effective services.  You’ll also have full responsibility for coordinating a range of services and their oversight.


We’re looking for someone with a minimum of 2 years of experience in human services, working with individuals with developmental disabilities. A Bachelor’s degree is preferred.  A typical background for this position may be someone who is a Direct Support Professional (DSP) or a Case Manager.


This position is a great opportunity to learn and grow your career in human services. You’ll work closely with other staff who will train you on various job responsibilities, including:

  • Creating service budgets
  • Working with families to hire staff
  • Understanding the State of NH In-Home Supports regulations
  • Developing service agreements
  • Negotiating with vendors
