Medication Support Specialist — Adults

Riverbend Community Mental Health, Inc., one of the largest providers of community-based mental health services in New Hampshire, is seeking a full-time Medication Support Specialist to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.

As a Riverbend Medication Support Specialist, your primary responsibility will be providing community-based medication support services designed to promote independence in the community.  Other responsibilities include the provision of community-based functional support services and limited case management.  Applicants must be flexible and comfortable working with a variety of challenges.  This is a great opportunity to work on the front lines of community-based mental health services and support our clients to achieve optimal recovery.  Team members have opportunities for professional growth, training and exposure to evidence-based practices. The anticipated hours will be 1:00 PM to 10:00 PM

Requirements:

• Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services is required.

• Excellent communication, problem-solving, time management, and organizational skills is required.

• A valid driver’s license, personal vehicle, and auto insurance is required.

Responsibilities:

• Help clients to understand medications and side effects

• Work with treatment team to facilitate case coordination and information sharing

• Provision of education regarding mental illness

• Work with clients to build symptom management skills and recovery strategies

• Promote effective utilization of clinical resources and other social supports
