Riverbend Community Mental Health

Clinician – Crisis Treatment Center ($5,000 Sign-On Bonus)

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Clinician to join our talented multi-disciplinary team in our Crisis Treatment Center.  The Clinician in this role works with clients (and their families/caregivers) who are experiencing behavioral health crises in the Concord Hospital Emergency Department and at the Crisis Treatment Center (CTC) to provide emergency assessment services and promote timely access to appropriate community-based care. This position is eligible for a $5,000 sign-on bonus!

  • Master’s degree in Psychology, Social Work, or related field required.
  • Eligibility for NH License as LCMHC, LICSW or LMFT preferred.
  • Minimum of two years’ work experience in a mental health treatment environment preferred.
  • Experience providing case management or care coordination services in an organization providing behavioral health, substance use disorder services, or emergency room services is preferred.
  • Valid driver’s license, personal vehicle, and auto insurance required.
  • Excellent communication, problem-solving, time management, and organizational skills are required.
  • Work with staff of Concord Hospital to provide rapid assessment and transfer to outpatient crisis services.
  • Conduct lethality assessments and determine overall lethality risk.
  • Develop appropriate dispositions which include specific referrals, follow-up care, and a plan for safety appropriate to the level of risk.
  • Develop a community care plan (emergency plan, health management plan, and ongoing action plan) with the client, family/caregiver(s) and providers, as appropriate.
  • Serve as the contact point, advocate, and informational resource for clients, care team, family/caregiver(s), and community resources.
  • Educate client and family/caregiver(s) about relevant community resources.
  • Facilitate and attend meetings between client, family/caregiver(s), and care team.
  • Educate Concord Hospital staff about the Crisis Treatment Center services.
