Discrete Case Manager - Adults

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Discrete Case Manager to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.

As a Riverbend Discrete Case Manager, you'll work closely with our clinical team to provide case management services for a large caseload of clients whose behavioral health needs consist primarily of office-based mental health interventions. The primary role of the Discrete Case Manager will be to assess needs, coordinate care and make referrals to community resources for his/her clients. The Case Manager will also support clinical processes associated with psychotherapy when applicable. Applicants need to be willing to learn and apply principles associated with Dialectical Behavioral Therapy. 

  • Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services is required
  • Master's degree in Psychology, Social Work, or related field preferred.
  • Valid driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative is required
  • Excellent communication, problem-solving, time management, and organizational skills is required
  • Help clients and families understand treatment options and make informed decisions
  • Work with treatment team to facilitate case coordination and information sharing
  • Participate in the development and implementation of client treatment goals
  • Work with clients to build symptom management skills
  • Promote effective utilization of clinical resources and other social supports
  • Care coordination/communication both internally and externally.
