Cornerstone VNA is a non-profit home, health and hospice organization serving Rockingham, Strafford, Belknap and Carroll Counties in New Hampshire and York County in Maine.  The team at the VNA uses the latest technology to provide the most highly skilled nursing, rehabilitative therapies, social work, and support services in their service area.

Certified Specialty programs include Wound, Ostomy & Incontinence care, Diabetes Management and Education, Certified Behavioral Health Nursing, Certified Intravenous Therapy, Chronic Care Management, Palliative Care Consultations, a Maternal Wellness Program and Fall Prevention & Balance Therapies.  Specialty services include a Telehealth Program, an in-home health monitoring system, and Smart Care, an emergency response and communication system for patients and their families providing 24 hour, 7 day a week peace of mind and added sense of security when making the transition to homecare.

Position Description

The nurse's responsibility includes collecting client health data and working with the patient/family in the patient's home. Must work closely with RN's and team to achieve goals. Will also be responsible for working with weekend supervisors to assist in all intake and scheduling duties as well as doing visits in the field. Office duties on the weekend include but are not limited to coordination of referrals, assisting in scheduling of visits, triaging calls. Weekend duties would be split between office and performing home visits as needed.

Qualified candidates must be committed to providing outstanding care and possess the following qualities: detailed oriented, honesty, flexibility in balancing office duties with clinical care in the field, positive easy going attitude, organized, open-minded, be a team player and have proficient computer skills. This position is flexible with days and hours between 32 hours and full time.


Educational Requirements:

  • Current NH Licensed Practical Nurse (LPN) 
  • ICD-10 coding experience, certification preferred (or will obtain upon hire)
  • Home care experience preferred


  • Computer competency, proficiency with medical EMR
  • Ability to provide trusted, compassionate and expert care
  • Flexibility to cover weekends/holidays/during vacations
  • Maintain confidentiality
  • Provide quality care in a timely manner
  • Ability to work in a team setting and individually
  • Medical knowledge/training preferred
  • Organization multiple tasks and prioritize simultaneously
  • Exceptional assessment skills
  • Strong communication skills
  • Valid driver's license in good standing and vehicle insurance are required
