Program Manager

North Country Health Consortium

The North Country Health Consortium (NCHC) is a dynamic and innovative non-profit organization created in 1997 as a vehicle for addressing common issues through collaboration among health and human service providers serving Northern New Hampshire. NCHC is engaged in activities for:

  • solving common problems and facilitating regional solutions
  • creating and facilitating services and programs to improve population health status
  • health professional training, continuing education and management services to encourage sustainability of the health care infrastructure
  • increasing capacity for local public health essential services
  • increasing access to health care for underserved and uninsured residents of Northern New Hampshire

The following Full-Time position is available within NCHC’s Wellness And Recovery Model (WARM) Program. WARM works to integrate recovery supports, providing care coordination and resource navigation, to enhance access to treatment and recovery for substance use disorder (SUD), and improve the knowledge and understanding of community members and professionals regarding best-practices around SUD.

Requirements:
  • Bachelor’s Degree in a related field and experience in the Substance Use Disorder field
  • Ability to work with and manage multi-disciplinary team
  • Strong organizational, interpersonal, writing and communication skills
  • Ability to set up and maintain efficient and effective systems and processes to manage details and grant deliverables
  • Willingness to travel within northern NH
Responsibilities:

• With a team-based approach: oversee coordination, professional development, and deployment of WARM Community Health Workers/Recovery Coaches • Supervised by the  Senior Program Manager, collaborate with Advisory Group members, Federally Qualified Health Centers, Critical Access Hospitals, Social Service Agencies, and other pertinent care coordination teams, staff treatment providers, and recovery resources to recognize the unique health needs for patients referred to the program
