Bereavement Coordinator

Concord Regional VNA is hiring for a part-time Bereavement Coordinator! Part-time benefits offered.

When you join the Concord Regional VNA staff, you have the opportunity to provide guidance and support during the most difficult time of a person’s life. 

The Bereavement Coordinator helps us to achieve our mission by providing and coordinating bereavement care of Hospice patients and families. The Bereavement Coordinator acts as the point of contact for community bereavement inquiries, coordinates grief groups and supervises bereavement volunteers. 

Essential Job Functions

Responds to community bereavement inquiries. Assists with resources and referral information.

Works effectively with members of the Hospice Team in establishing, implementing, and evaluating the care plan for each family regarding bereavement needs.

Coordinates/offers bereavement support to facilities and offer community education and outreach on grief and loss when requested.

Coordinates and provides grief and loss counseling according to identified needs. (Schedules phone calls and in-person visits).

Effectively documents bereavement services in the patient-bereaved record according to procedure.

Reviews and updates the patient/family bereavement assessment tools, hand-outs and educational materials at least annually.

Works with the Director to train bereavement volunteers to facilitate bereavement groups as appropriate.

Plans, coordinates, and implements in-service education for Agency staff and Bereavement Volunteers that responds to current needs and trends.

Coordinates evaluation of bereavement and tracks patient and family satisfaction results as it relates to bereavement.

Prepare new reports and review existing reports regarding bereavement services. Interpret results for program planning.

Assigns and supervises volunteers to bereavement cases facilitating groups and phone calling as appropriate. 

Offers community education and outreach on grief and loss when requested.

Completes annual bereavement volunteer competencies.

Communicates with all physicians involved in the POC and other health practitioners related to the current POC.

Plans for, organizes and coordinates bereavement groups to meet the needs of caseload and community. 

Plans and organizes community memorial services.

Requirements:

1. Master's Degree in Social Work, Counseling or Divinity

2. 2-4 years of experience in the field or in a related area

Work Environment

The employee will be exposed to a variety of uncontrolled work environments. The employee may be exposed to all kinds of weather conditions while driving to and from work assignments.  The employee may also be exposed to blood and bodily fluids, virus, disease and infection from patients and environmental hazards in the work environment.  The employee may be required to work extended periods of time using a laptop and/or other mobile device.  

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential function of this job. This position requires the ability to frequently enter and exit and/or sit in an automobile for prolonged periods while commuting to work assignments.
