Case Manager and Advocate

One Sky Community Services

One Sky Community Services serves Region VIII; one of ten regions in New Hampshire’s nationally acclaimed support system for individuals with developmental disabilities. This system enables individuals who were once placed in an institution to live more independently and pursue satisfying lives in their local communities.

Area agencies contract with the NH Bureau of Developmental Services (BDS) and are subject to state contract agreements and regulations. The agencies are funded primarily by state and federal Medicaid dollars, each one maintaining regional autonomy and local control. At the same time, area agencies share a common vision with the BDS and work closely with it to sustain the community-based system of services.

One Sky Community Services is looking for a Case Manager to enrich the lives of our clients!

One Sky Community Services is a local private non-profit that serves individuals living with developmental disabilities and acquired brain injuries in Rockingham County. We work to help the individuals we serve to live as valued and participating members of the community by:

  • Helping people make choices for themselves about where they live, work and spend time
  • Using existing resources when possible, but also creating and maintaining supports and services which are flexible and responsive to the circumstances of each individual
  • Assisting families in ways they view as supportive
  • Promoting community understanding, acceptance, and support of those we serve
  • Fostering inclusion in community affairs and relationships
  • Working in cooperation with local citizens and professionals to realize this mission
  • Providing services with regard to race, sex, religion, national origin or political belief

The case manager in this position will be responsible for individuals who have elected to join our family/participant directed services. This type of case management focuses on the needs and desires of the individuals we serve and gives our clients and their guardians/ family a greater level of autonomy in the services they receive.

This is a full-time position which includes eligibility for our organization’s medical and dental plans, 11 paid holidays, and four weeks’ of paid time off your first year with the agency!

Requirements:
  • Bachelor’s degree or equivalent in Human Services or related field
  • Experience in related position with responsibility for developing community, family and client relationships, creating service plans, negotiating and budgeting resources, quality assurance, compliance, or other comparable areas.
  • A real desire to make a difference in the lives of others.
  • Creative ability to solve problems and “think outside the box” to acquire solutions and resources for our clients.
  • Must be able to physically access family homes, community buildings, program sites, etc.
  • Must provide or have available own transportation. A criminal record, driver record and proof of auto insurance coverage of 100/300 is required; employment is contingent on satisfactory results.
Responsibilities:
  • Identifying the needs, goals, and aspirations of the individuals we serve.
  • Assessing each individual’s capabilities to achieve those dreams on their own
  • Identifying the gaps in each individual’s capabilities/resources
  • Developing and leading the effort to implement a plan that addresses those gaps; which includes:
  • Prioritizing and negotiating needed services in ways that are appropriate, cost effective and quality driven;
  • Ensuring that all services are delivered in compliance with agreed upon service agreements, budgets and consistent with all applicable rules and regulations
