Intake Coordinator

North Country Health Consortium

The North Country Health Consortium (NCHC) is a dynamic and innovative non-profit organization created in 1997 as a vehicle for addressing common issues through collaboration among health and human service providers serving Northern New Hampshire. NCHC is engaged in activities for:

  • solving common problems and facilitating regional solutions
  • creating and facilitating services and programs to improve population health status
  • health professional training, continuing education and management services to encourage sustainability of the health care infrastructure
  • increasing capacity for local public health essential services
  • increasing access to health care for underserved and uninsured residents of Northern New Hampshire

The following  Intake Coordinator position is available within NCHC’s Substance Use Disorder Clinical Services (at the Friendship House location) that serves individuals with substance use and co-occurring mental health disorders.

Requirements:

QUALIFICATIONS/REQUIREMENTS 

  • Bachelor’s Degree in Human Services or minimum of one-year supervised experience as a residential treatment program admissions coordinator
  • Ability to perform job responsibilities under stressful conditions within a fast-paced environment
  • Working knowledge of the disease of drug dependence, progression of chemical dependency, the treatment continuum of care, and referral sources
  • Empathetic and professional demeanor to support individuals seeking help for a substance use disorder and their family members
  • Excellent observation, written & oral communication skills
  • Fluency with computer programs and Microsoft Office Suite, especially email and calendar scheduling
  • Valid Driver’s license and proof of automobile insurance coverage
Responsibilities:

JOB DUTIES

  • Field phone calls and correspondence pertaining to the residential treatment program • Establish and conduct intake phone interviews with potential clients • Conduct initial screenings for potential residents to determine clients’ need for treatment and level of care, consulting with the supervisor as necessary • Facilitate pre-admissions process with admitting clients • Provide clear communication with clinical and administrative staff  • Manage and maintain wait-list • Initiate and maintain clinical charts/files, managing the confidential exchange of client information • Reporting and documentation • Establish and maintain positive working relationships with referral sources, including social workers, probation officers, and medical providers • Assist with administrative tasks
