Intake Specialist/Family Support Program

Family Support Intake Specialist, Manchester, NH.  

As the Family Support Intake Specialist you will oversee the intake process and conduct assessments for all appropriate referrals to the Parenting Plus program. The FSIS will focus on understanding Parent/Child Interaction and family dynamics that promotes optimum social emotional and physical development of children.  While working in partnership with schools, state agencies, and medical providers as well as social service agencies to address families concrete needs and to help promote family self sufficiency. The Intake Specialist will work closely with the site manager to ensure families receive appropriate services based on the administered assessments in order to provide a cohesive transition of services to the family's ongoing worker when needed. Additional responsibilities include completing all required documentation to meet the guideline of Medicaid billing, working with Parent Plus team to ensure transition of services for families, and maintaining data bases. BA or equivalent preferred with a minimum of 1-2 year related experience. The successful candidate will have excellent organizational skills, knowledge of child development, knowledge of assessments, an eye for details, strong writing skills, and have strong customer service skills. Previous experience with Office 365 and Sharepoint strongly desired. The chosen candidate will also be required to submit to a criminal background, motor vehicle record check and have auto insurance that meets agency standards.
