Family Support Intake Specialist

Family Support Intake Specialist, Manchester/Nashua, NH As the Family Support Intake Specialist you will oversee the intake process and conduct assessments for all appropriate referrals to the Parenting Plus program. The Intake Specialist will be responsible to engage and meet with the families in their homes or community to complete all essential opening paperwork as well as required evidenced based and supported assessment tools. The Intake Specialist will work closely with the site managers and family resource center to ensure families receive appropriate services based on the administered assessments in order to provide a cohesive transition of services to the family's ongoing worker. Additional responsibilities include completing all required documentation to meet the guideline of Medicaid billing, working with Parent Plus team to ensure transition of services for families, and maintaining data bases.  This is a full time position serving the Manchester and Nashua areas and includes a comprehensive benefits package. BA or equivalent preferred with a minimum of 5 years related experience to include specific experience in comprehensive family support programs. The successful candidate will have excellent organizational skills, knowledge of child development, knowledge of assessments, an eye for details, strong writing skills, and have strong customer service skills. Previous experience with Office 365 and Sharepoint strongly desired. The chosen candidate will also be required to submit to a criminal background, motor vehicle record check and have auto insurance that meets agency standards.
