TLC Family Resource Center

TLC Family Resource Center is a family resource and support agency located in Claremont, New Hampshire.  The mission of TLC Family is to promote the optimal health and development of New Hampshire children and families.  The agency offers health education, parent education, case management and support services for pregnant women, infants, children and families living in the Claremont and Lebanon region.  

TLC Family Resource Center helps families in a manner than is non-judgemental, respectful and confidential.  Our parent educators, nurses and mental health counselors work with families in their homes and in the community to ensure the physical, cognitive, social and emotional health of their children.  

Our philosophy is based on the premise that prevention is better than intervention, and that all families deserve voluntary and timely access to family health and support services. We believe that families are the experts about their own lives, and that it's important to bring laughter and joy into our work with children and families.  Our staff understand that the development of a trusting, respectful, genuine caring relationship between parents and those who help them is essential.


Position Description

Job Title:                    Parent Educator/Home Visitor

Basic Function:         The mission of TLC Family Resource Center is to promote the optimal health and development of New Hampshire children and families, specifically those living in Sullivan County.  In order to accomplish our mission, TLC employs parent educators who help to strengthen and support families.  The role of the parent educator is to help parents and children develop healthy attachment, create positive parent/child relationships, and help families to achieve economic, physical, social, and emotional security.

Essential Duties:

  • Establish professional relationship with referred families.
  • Make home visits to families to provide specific interventions designed to promote the development of healthy family relationships.
  • Assess family strengths and areas of concern.
  • Help families to develop goals for themselves.
  • Provide care coordination, assisting families to access community resources.
  • Write and maintain a current Individualized Family Support Plan for each family.
  • Assist families to implement action steps to meet their goals.
  • Participate in team meetings and individual/group supervision with clinical supervisor.
  • Maintain and submit timely documentation as required by supervisor.
  • Participate in initial and ongoing training to enhance effectiveness and knowledge of family support principles and techniques. 
  • Meet productivity goals (% of direct service hours per FTE).

Reports to and is supervised by the Executive Director and Clinical Supervisor

Requirements:
  • BA in early childhood or related subject
  • 2-3 years experience working with young children and families
