Case Manager - Elder Adults

Case Manager – Elder Adults

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Elders Case Manager to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.

As a Riverbend Case Manager, your primary responsibilities will be working closely with clients to coordinate treatment, assisting clients in obtaining the services they need and facilitating communication with the treatment team within Riverbend and with outside agencies.  Coordination of services and ensuring the flow of communication between multiple providers is necessary given the complex medical/psychiatric needs of our clients. This is a great opportunity to work on the front lines of community-based mental health services, and help our clients achieve optimal recovery and independence.

Responsibilities

  • Help clients and family understand treatment options and make informed decisions
  • Work with treatment team to facilitate case coordination and information sharing
  • Participate in the development and implementation of client treatment goals
  • Work with clients to build symptom management skills
  • Promote effective utilization of clinical resources and other social supports
  • Coordination of services between multiple medical/psychiatric providers
Requirements:
  • Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services is required.
  • A valid driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative is required.
