Case Manager

Cross Roads House

Cross Roads House provides shelter and services to homeless individuals and families in the Greater Seacoast area.  We provide both emergency shelter and a transitional program to help our residents move with dignity and purpose to stable and decent housing.  

The Shelter Case Manager works as part of a comprehensive social work team to provide intensive service coordination and suppport services to individuals and families at Cross Roads House,  helping them to address barriers to self-sufficiency and to facilitate their timely return to stable housing.  


Position requires at least a Bachelor’s degree in social work or other relevant field and at least three years of experience providing service coordination and support to people experiencing poverty or homelessness.  Looking for candidates with excellent communication, organization and crisis management skills; a positive regard for others; and an enthusiastic drive to make a difference.

  • Engage and maintain regular contact with individual and family residents;
  • Assess residents for strengths and barriers, including housing, employment, mental health, substance use, and criminal history
  • Collaboratively assist residents with establishing housing and self-sufficiency goals, identifying appropriate steps towards achieving goals, modifying goals as needed, and tracking progress
  • Provide community resource referrals, coordinate with service providers, advocate on behalf of residents as appropriate
  • Provide encouragement and guidance to residents; set and maintain expectations for progress
  • Support and assist residents with housing and job search, budgeting and financial management practices, banking and credit repair, and self-advocacy
  • Assess crisis situations and respond appropriately
  • Maintain precise and accurate documentation of case management services, including paper and electronic records
  • Adhere to professional and ethical guidelines, including maintaining appropriate boundaries with residents and ensuring appropriate confidentiality
  • Collaborate with other staff members so that individual resident needs are effectively addressed
  • Provide qualitative and quantitative information about resident progress, needs, and program outcomes 
  • Recommend and support improvements in program design and shelter operations, as appropriate
  • Participate in local and regional collaborative efforts and represent Cross Roads House as needed
