We strive to make working with us as simple as possible. Lakes Region VNA accepts payment from Medicare, Medicaid, Veterans Assistance, Workers' Compensation, private pay, and private insurance. We never refuse patients due to the inability to pay. If we can't provide the care you need, we'll help you find a suitable alternative.

About the Job
Lakes Region Visiting Nurse Association, located in Meredith, NH, is a New Hampshire-licensed home care and hospice agency that offers a full range of home care and hospice services. Each year, we provide home care services to over 35 towns in central New Hampshire and complete approximately 40,000 annual visits to our community residents in the comfort of their homes.
As a semi-rural patient-centered agency, we offer an opportunity to work in a fast-paced and friendly work environment where the providers of care really get to know their patients and their families. The staff at LRVNA take great pride in being able to promote health and independence across the lifespan through our skilled care services and education of patients, families, and caregivers.
LRVNA provides professional and comprehensive home care through an integrated team of nursing, physical therapy, occupational therapy, speech therapy and social work professionals. The team works with physicians, advanced practice nurses and hospital staff to develop individualized care plans to optimize health maintenance, recovery, chronic care, and independence. The hospice program provides comprehensive care at the end-of-life as well as grief support for families. Services are available 7 days a week with on-call staff, 24 hours a day.
We have FT, PT and Per Diem hours. You will provide skilled nursing care to patients in their own environment. Become an integral part of a dynamic, multi-disciplinary team while maintaining a high level of autonomy. Enjoy a flexible workday, with an option to start and end your day from home. Strong Clinical support and educational resources provided. Home Care experience beneficial, although we will train the right individual.
Excellent Salary and Benefit package available for FT with a guaranteed 30 hours per week, on call required.
Primary Duties and Responsibilities:
- Completes a comprehensive initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs.
- Initiates and develops the plan of care which establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Makes necessary revisions as patient status and needs change.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Assessment, Education, and Administration of medications and treatments as prescribed by the physician.
- Provides health care instructions to the patient and families as appropriate per assessment and plan.
- Evaluates for other discipline needs, obtains orders and collaborates on care plan.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
