We work with community, state and federal partners to assist children, families, low-income and elderly residents with their efforts to become or remain financially and socially independent through a variety of coordinated programs. GENERAL DESCRIPTION
CAPSC is seeking a compassionate and resolute individual to join our team as a DV Housing Specialist. In this role, you will work closely with individuals and families fleeing and attempting to flee DV and experiencing or at risk of homelessness. You will provide comprehensive case management services aimed at addressing their immediate needs and facilitating their journey towards stable housing and self-sufficiency.
The DV Housing Specialist will provide direction and guidance on the content of work and oversee the quality and scope of programs for many clients with a variety of needs. This position is ultimately responsible for ensuring services to families and individuals are of the highest quality possible. The DV Housing Specialist will engage weekly with clients throughout the program from referral until discharge. This will be done in person and/or virtually. They will determine eligibility for the DV housing programs, complete enrollment process, assess their needs, and refer to local resources as needed.
EDUCATION and/or EXPERIENCE
- B.A./B.S. degree in social services or related field.
- Or A.A./A.S. degree and 2-3 years of previous experience working with individuals experiencing homelessness, preferably in case management or direct service capacity.
- Knowledge of homeless and/or domestic violence systems/HUD
- Strong computer skills, including proficiency in Microsoft Office products and ability to quickly learn program-specific software programs.
- Experience working independently, remotely, as part of a team, and in conjunction with local resources
- Strong time management skills, professional demeanor, and attention to confidentiality.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Maintain a consistent and effective program, ensuring positive staff/client communication and overall staff/client satisfaction.
- Complete training with a local agency to comply with RSA:153b.
- Maintain consistent contact with clients, including in person meetings. Provide transportation for clients when deemed necessary, utilizing own transportation or CAP transportation when available.
- Develop individualized service plans in collaboration with clients, focusing on short-term and long-terms goals for safety, housing stability, and personal growth.
- Offer supportive coaching and provide advocacy to empower clients in navigating the complexities of homelessness and accessing available resources and services.
- Coordinate with external agencies, including NHCADSV agencies, shelters, housing authorities, healthcare providers, schools, and social service organizations, to facilitate access to necessary support services for clients.
- Ensure timely service and support to clients, such as referrals for other services, conducting a needs evaluation and resolving complaints.
- Assist clients with applications for SNAP, Medicaid, TANF, VA or any applicable benefit programs.
- Assist clients in applying for and securing housing opportunities, including transitional housing, permanent supportive housing, and rental assistance programs.
- Advocate on behalf of clients to address barriers to housing, employment, healthcare, and other essential needs.
- Act as a resource to agency staff as well as other community programs regarding program related federal, state, and county regulations and policies related to community services programming.
- Maintain accurate and up-to-date documentation of client interactions, services plans, referrals, and outcomes in compliance with agency policies and funding requirements.
- Represent the agency at local, state, and national meetings as required.
- Participate in staff meetings, training sessions, and professional development activities to enhance knowledge and skills in serving individuals experiencing homelessness.
- Assist DV Housing Support Manager in preparing grant and funder reports.
- Ability to work flexible schedule as required.
