Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission. We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.

Job Summary: The Clinical Coordinator will provide supervision and mentoring for clinical staff and administrative oversight of the Assertive Community Treatment Team (ACT) and Older Adult and Neurocognitive Team (Neuro), while collaborating and communicating with community partners on program monitoring and development. Provide assessment, individual, family and group therapy, and crisis intervention to patients.
Essential Duties/Responsibilities:
· Facilitates team meetings in collaboration with the team Prescriber, promotes team cooperation and integration to create effective team functioning and communication and stimulate team growth.
· Provide individual supervision for all assigned staff, including Neuro Clinicians and all ACT staff. Educates and orients staff to program philosophy/values and goals to ensure understanding of, commitment to, and cohesiveness of program goals and vision.
· Demonstrates a working knowledge of the ACT fidelity scale and implements best practices to ensure strong adherence to all fidelity measures.
· Oversees the referrals to the team and assigns staff in a timely manner.
· Assures quality and timely documentation on records/paperwork of staff on their teams according to established standards.
· Develops and maintains effective therapeutic relationships with a diverse caseload of patients, develops and implements individual service plans for each patient, conducts assessments as needed, and completes all quarterly and annual paperwork.
· Provides clinical support, crisis intervention and other direct care to facilitate team collaboration.
· In conjunction with the Director and Assistant Director, has responsibility for recruiting new staff members, to include screening candidates, participating in interview sessions and assisting in the hiring process.
· Has general knowledge of the program’s budget and monitors financial and statistical data to manage the program’s costs, revenues, and overall service delivery.
· Participates in community-based organizations as a representative of Lakes Region Mental Health and participates on agency committees.
· Understand the philosophy and operating procedures of LRMHC and apply them effectively both within and outside the agency.
· Attendance at state wide meetings representing LRMHC for Assertive Community Treatment and liaison to Lakes Region Community Services.
· Performs all other duties as requested by the Program Director.
· Monday through Friday with occasional requirements to cover emergencies after hours.

· Takes a person-centered approach to treatment, as noted through supervision, team participation, and feedback from patients and staff.
· Provides input into program issues and shares/receives feedback in a healthy, constructive atmosphere.
· With the assistance of the Program Director, is able to organize and prioritize a workload so that administrative duties are completed efficiently and in timely manner.
· Represents the patients/program/agency constructively both within and outside the agency
· Ability to express or exchange ideas by means of the spoken word and to receive detailed information through oral communication. Primary functions include activities in which verbal communication with patients and with others is key to job effectiveness.
· Visual acuity must meet a minimum standard to work with preparing, reading and analyzing data and reports, work with computer terminals, etc.
Education and Experience:
· Master’s in mental health counseling, social work, psychology, nursing, counseling or related field as specified in He-M426-11 and He-M426-13 with 2 to 3 years’ experience in mental health, crisis intervention or medical setting or related position, offering direct mental health services.
· Licensed or license eligible as LCMHC, LICSW or MFT in NH. Must possess a valid NH driver’s license.
