Psychiatric Care Manager

$2,000 SIGN ON BONUS

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Psychiatric Care Manager to provide timely, accurate and high quality care management services for individuals awaiting psychiatric inpatient admission and disposition in Concord Hospital’s Emergency Room.

As the COVID-19 pandemic continues to impact the world, it is our responsibility as healthcare providers to help keep our staff, clients, and communities safe. At Riverbend, we have implemented processes and precautions to help prevent the spread of COVID-19 as well as a mandatory vaccination policy.  Exceptions can be applied for under medical or religious reasons. Additionally, weekly testing will be required for anyone who is in progress of the vaccine series or individuals with approved exceptions.

Requirements:
  • Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services required.
  • Valid driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative is required.
  • Excellent communication, problem-solving, time management, and organizational skills.
  • Must possess an understanding of the inpatient and outpatient systems of care across the lifespan.
  • Ability to obtain and maintain Concord Hospital credentials is required.
Responsibilities:
  • Participate in daily psychiatric rounds to best support patient disposition
  • Obtain pertinent releases of information (ROI)
  • Communicate with internal and external providers to ensure continuity of care
  • Support the hospital admission process through the maintenance of electronic communication systems
  • Facilitate inpatient placement through the organization and transfer of admission records and labs
  • Work closely with medical providers to acquire additional labs and tests, as needed
  • Coordinate the receipt of all relevant client data, files, and collateral communication across systems
  • Track inpatient queue and facilitate placement & discharge which includes completion of insurance authorization & Emergency Medical Treatment and Labor Act
  • Coordinate meetings with guardian, family, and providers to improve patient outcomes
  • Collaborate with internal providers to ensure the timely placement or discharge of patients
  • Promote effective utilization of clinical resources and other social supports
