Case Manager – Child, Adolescent & Family Services

Established in 1920, Greater Nashua Mental Health has as its mission: empowering individuals to lead full and satisfying lives through treatment and support. Our agency provides comprehensive, evidence-based behavioral health services for individuals with mental health and/or substance misuse disorders. We offer a wide array of services in office and community settings, serving adults and children.  As the State-designated Community Mental Health Center for Southern Hillsborough County, we serve everyone who walks in our doors, and no one is ever turned away due to the inability to pay for services.  Without our agency, many of the most vulnerable members of our community would not have access to critically needed behavioral health care.   We believe that by respecting people as they are, we can help them to become all that they wish to be.

Job Summary:

Greater Nashua Mental Health is looking for a full-time Case Manager to join our Child, Adolescent & Family Services Team! The Case Manager provides direct support, education, referral, and outreach to a caseload of children and adolescents and families diagnosed with severe mental illness. The Case Manager meets with individuals in their natural environments and provides case management services necessary to facilitate empowering clients and families to lead full and satisfying lives. Additionally, the Case Manager provides Functional Support Services (FSS), which are rehabilitative services, and environmental supports, which sustain the client in his or her current living situation or other community settings.

Duties & Responsibilities:

  • Provides Crisis Intervention Services to individuals in order to stabilize acute behaviors and symptoms.
  • Delivers specific and individualized Therapeutic Behavioral Services to individuals, families and groups to achieve the client's optimal level of functioning.
  • Provides Family Support to support and maintain the management of the client's mental illness or serious emotional disturbance, with the primary objective to enhance and promote the client's resiliency and recovery.
  • Works collaboratively with clients and families and treatment team members to formulate care and support plans that maximize the individual's control and choice over how their care needs are met.
  • Links and refers clients and families to needed resources outside of the mental health center, including but not limited to medical, educational, and social services.


  • Bachelor’s degree in social work, psychology, rehabilitation, education or a related human services field. Or, an associate’s degree in social work, psychology, rehabilitation, education or a related human service field along with either two years of experience working with persons who have a serious emotional disturbance or serious mental illness, or two years of experience that provides an understanding of mental illness. Applicants that are in process of obtaining a degree may also be considered.
  • Knowledge of children’s mental health issues, developmental issues & family systems.
  • Strong collaborating skills and an ability to work effectively both independently and as part of a team.
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
