Engagement Specialist

Lakes Region Mental Health Center

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.


Bachelor’s degree in Social Work, Psychology, Counseling, Human Services or other related field. One or more years experience with emphasis in mental health. Valid NH driver’s license.


•Follow up with all patients who have missed appointments.
•Assign and provide patients with medication clinic and /or groups to assist in re-engaging patient.
•Provide discharge and/or transfer planning for patients that cannot re-engage.
•Screen and evaluate risks of patients referred for services, and intervene, stabilize and manage acute crisis situations
•Screen and evaluate referrals and evaluate patient needs, strengths, goals and resources.
•Refer and link patients to community resources and services if patient is unable to re-engage in treatment.
•Work with interdisciplinary team in reviewing cases and developing Individual Service Plans to reengage clients into services.
•Work with Hospital Liaison to provide linkage case management, participate in treatment planning and discharge planning for patient being discharged with history of non-engagement with treatment.
•Consult with Treatment Teams and Prescribers regarding assessments for non-engagement and service plan development.
•Consult with other community service agencies around case-specific and programmatic services.
