Case Manager, Integrated Dual Disorder Treatment Team

Job Summary:
Greater Nashua Mental Health is looking for a full-time Case Manager to join our Integrated Dual Disorder Treatment Team! The Case Manager provides direct and outreach services, including therapeutic behavioral services, medication supports, family supports, and crisis intervention services, to a caseload of individuals who have co-occurring severe mental illness and substance use disorders.

Duties & Responsibilities:
• Reviews all cases assigned to the Case Manager to familiarize self with client needs and history; assesses the need for further evaluations and review of interim treatment plan.  Provides clinical assessments of clients, as needed.
• Coordinates referrals for, and reviews results of additional assessments, as appropriate.
• Provides clinical input and formulation of IDDT Treatment Plan, which identifies specific problems to be addressed, target goals related to problems, and clear and measurable objectives with clear time frames attached to specific services to be provided.
• Reviews and updates treatment plans to ensure that they represent current and appropriate planning which complies with GNMH guidelines.
• Provides treatment to consumers and families according to the established treatment plans, utilizing approved modalities of the agency, for a percentage of time per week as appropriate for position and as directed by supervisor.
• Assesses crises and provides crisis intervention and, in coordination with the client's primary psychiatrist, develops referrals for more or less intensive services appropriate to consumer need.
• Coordinates the consumer's Individual Service Plan (ISP) within the Agency and with eternal agencies; communicates, collaborates, and consults with other staff and agencies.  Ensures the ISP services are carried out when the consumer allows.
• Completes all Quarterly Reviews within a two-week window of the end date of the quarter being reviewed.  Completes all other documentation within 3 business days (with first day being date of service/completion).

• Bachelor’s Degree in Psychology, Social Work or related field.
• 2-3 years of experience working with people diagnosed with substance use disorders preferred.
• Familiarity with community resources.
• Knowledge of and ability to use Microsoft Office products (Word, Outlook, Excel, etc.).
• Strong collaborating skills and an ability to work effectively both independently and as part of a team.
• Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
