Case Manager - Adults

Founded in 1963, Riverbend Community Mental Health is a private, nonprofit organization providing specialized behavioral health services in central New Hampshire. Core services include 24/7 emergency mobile crisis services; counseling services for children, adults, and families; addiction treatment; and community support, vocational services, and residential programs for adults and seniors. Case Manager - Adults ($1,500 sign-on bonus)

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Case Manager to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.  *This position offers a $1,500 sign-on bonus!

As a Case Manager, your primary responsibilities will be working with clients to provide case management and outreach services. You will work closely with treatment teams within Riverbend as well as outside agencies. This team serves primarily young adults and/or individuals with intellectual disabilities. This is a great opportunity to work on the front lines of community-based behavioral health services, and help our clients achieve optimal recovery and independence.

This is a full-time position with a competitive salary and an outstanding benefits package including:

As the COVID-19 pandemic continues to impact the world, it is our responsibility as healthcare providers to help keep our staff, clients and communities safe.  At Riverbend, we have implemented processes and precautions to help prevent the spread of COVID-19 as well as a mandatory vaccination policy.  Exceptions can be applied for under medical or religious reasons.  Additionally, weekly testing will be required for anyone who is in progress of the vaccine series or individuals with approved exceptions.

Requirements:
  • Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, Human Services or a related field is required
  • Valid driver’s license, personal vehicle, and auto insurance is required
Responsibilities:
  • Help clients and families understand treatment options and make informed decisions
  • Work with treatment team to facilitate case coordination and information sharing
  • Participate in the development and implementation of client treatment goals
  • Work with clients to build symptom management skills
  • Promote effective utilization of clinical resources and other social supports
