Bridge Program Case Manager

Lakes Region Mental Health Center

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.

The Bridge Program CCM will be responsible for implementing and supporting the Housing Bridge Subsidy Program. This position will serve as a liaison between Lakes Region Mental Health Center (LRMHC) and both the Bridge House and Speare Memorial Hospital, in an effort to increase access to services and reduce emergency room utilization due to homelessness.  The CCM will work collaboratively with staff, agencies and service providers to develop and provide stable, affordable, and long-term independent housing opportunities through short and long-term subsidies such as the Housing Bridge Subsidy Program, Housing Choice Voucher program or other types of permanent housing. The CCM will provide assistance to adults with serious and persistent mental illnesses who may experience homelessness.  They will provide support with a recovery focus to patients; and will assist patients to discover and maintain a valued role(s) in the community, school and home


The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how the individual can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities.  Must also posses the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

1.   Must be able to establish a rapport with the patients.  Must treat all patients with dignity and    respect.
2.    Has general knowledge about medication and diagnosis as well as a working knowledge of rehabilitation and supportive living concepts, and uses these appropriately with patients as observed by supervisor, patient, and team.
3.    Knowledge of the surrounding area and/or the ability to familiarize themselves with the surrounding area to support the patient in reaching their goals and objectives.
4.    Ability to organize work-load so that his/her clinical and administrative duties are completed in a timely and thorough manner without the need for direct supervision.
5.   Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts.
6.   Must be capable of working closely with other agency staff to expand the knowledge of mental health services both within LTSS Housing Program and across to other programs.  Must be able to develop strong working relationships with external services.
7.   Visual acuity must meet a minimum standard to work with preparing, reading, and analyzing data and reports, work with a computer terminal, etc.
8.   Is able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 lbs. or more.
9.Is able to tolerate periods of heavy workload or excessive stress.



Education: Bachelor’s in mental health counseling, social work, psychology, nursing, counseling or other related field as specified in HeM426-11 and HeM426-13. Meets qualifications to provide Functional Support Services.

Experience: One to two years of experience with emphasis in mental health. 

License/Certification: Must have and maintain a valid NH driver’s license and liability Insurance.

Work Schedule:

Generally works Monday through Friday with occasional requirements to cover emergencies after hours.


1.Assist individuals to secure and maintain affordable housing utilizing search activities such as contacting prospective landlords to schedule showings, scheduling housing interviews for individuals, assisting with the rental application process, and helping individuals make informed decisions. 
1.Provide housing assistance to ensure successful execution of a subsidy contract and to maintain current information about individual’s benefits to ensure that subsidy amounts are accurate with regard to individual’s income.
2.Provide housing advocacy and other support services to individuals.
3.Serve as liaison for the Bridge House and Speare Memorial Hospital, following up on referrals and facilitating access to care.  Additionally, will attend Agency Council on a monthly basis to assist community collaboration.
4.Prepare reports which may include demographic information, services provided to individuals, and appointments with individuals, landlords, property managers, housing subsidy administrators, and marketing efforts.
5.   Performs case management services for patients of LRMHC as defined by the administrative rules known as HeM’s in assisting individuals in gaining access to needed medical, social, educational and other services.
6.Completes case management assessment for LRMHC patients and develops comprehensive care plans, may do individual treatment plans, and monitors services received.  Completes required documentation in a timely and thorough manner and according to agency standards.  Maintains standards of productivity as established for the position.
7.  In collaboration with Prescriber, Emergency Services, families and natural supports, provides ongoing assessment, monitoring, support and crisis interventions to LRMH patients as needed.
8.Participates actively in supervision, team meetings, client centered conferences and Individual Service Planning (ISP) for LRMHC patients; makes contributions and applies what is learned effectively.
9.Monitors medications prescribed to LRMHC patients in the program as required and monitors compliance, provides education on prescribed medication and remains familiar with medications prescribed.
10.Establishes a staff development plan with supervisor and makes a concerted effort to seek training into areas identified in the plan.  Provides schedule, activities planned and outcomes and priorities within EMR and to supervisor.
11.Required to transport patients in their personal vehicle.
12.May perform other related job duties as assigned by their supervisor.
13.Attendance is a requirement of this position.
