NFI North's Care Management Entity has an opening for an Intake Coordinator to assist their state wide programs (Transitional Enhanced Care Coordination, FAST Forward and Community Based Services). This position will work in collaboration withv arious Program Directos and their staff as well as CME Administrators within offices in both Tilton and Littleton, NH.  This is based primarily in the Tilton office.

Requirements:

The candidate would be detail oriented, flexible and able to multitask, have excellent computer skills to include but not limited to expertise in Microsoft Word, great typing skills, Microsoft Excel, experience with electronic filing systems and able to collaborate well with outside resources.

Responsibilities:

Maintain office functions, meet and great visitors at office sites when participating in the telecommuting office days. Sends and receives correspondence, electronic filing, creating, maintaining and organizing documentation, managing spreadsheets and tracking forms. Conducts initial Plans of Care in Process with families, intra-agency client transfers between CME programs, manages and performs timely intake processes on referrals, including completion and submission of required paperwork, internal and external releases, permissions and authorizations, as well as providing proper Notification of insurance providers.  Provides assistance in tracking enrollments and disharges, supervising timelines by considering program, contact, state and other sources' guidelines. Prepares and maintains accurate participant tracking database and census reports as well as updates in electronic filing system as needed.
