Harbor Care, formerly known as Harbor Homes and the Partnership for Successful Living affiliates, is an innovative health and human services non-profit organization that provides housing, healthcare, home health, behavioral health, addiction treatment, and more in collaboration with many community partners.  Harbor Care is the new shared name of Harbor Homes, Keystone Hall, Healthy at Home, the Harbor Care Health and Wellness Center, and the Southern NH HIV/AIDS Task Force. We offer many programs specifically focused on our Military Veteran and homeless populations.  Harbor Care integrates stable housing with vital supports such as: primary, dental, and mental health care, substance misuse treatment, employment services, and other wraparound supports proven to end homelessness, help change and save lives.

About the Health and Wellness Center:

Harbor Care Health and Wellness Center offers primary, preventive medical services, behavioral health services, and dental services to children and adults who are homeless or at risk of homelessness, uninsured, under-insured, or low income.   Harbor Care offers a sliding scale fee for self-pay patients, and accepts Medicaid, Medicare, as well as most commercial insurances. The Southern NH HIV/AIDS Task Force has a long history of providing culturally sensitive support services to low-income individuals living with HIV/AIDS. With medical case management at the heart of our program, each client is assisted in creating a service plan with action steps leading to viral suppression while meeting their other basic needs.

 

Job Summary:

  • Harbor Care seeks a Physician to work as a key member of a highly accomplished medical team. The physician will have extensive training in internal or family medicine with a specialty in the diagnosis and treatment of infectious diseases to provide the following services at Harbor Care’s Health and Wellness Clinic:
  • Provide medical guidance and act as the subject matter expert, particularly on infectious disease or epidemiology-based events such an epidemic, outbreak, cluster or pandemic.
  • Provide medical diagnosis and post-exposure measures such as antibiotics, etc.
  • Maintain working relationships with other medical providers and/or agencies engaged in assessment and disease control efforts.
  • Collaborate with medical case managers as a team member of Southern NH HIV/AIDS Task Force.
Requirements:

Basic Qualifications:

  • Doctor of Medicine degree – previous experience with treating infectious diseases preferred.
  • Passed the United States Medical Licensing Exam.
  • Current registration and licensure in the State of New Hampshire or ability to obtain a license in New Hampshire.
  • Minimum of 3 years’ experience in Inpatient or Outpatient counseling.
  • Successfully complete all required company in-services.

 

 Preferred Qualifications:  

  • Knowledge of The Federally Qualified Health Center (FQHC) Regulations is a plus.
  • Multilingual is a plus (Spanish preferred but not required).
  • Knowledge of computers and software applications or ability to learn quickly.
  • Experience working with people from diverse populations.
  • Ability to work as an effective team member is essential.

 

Knowledge, Skills and Abilities:

  • Be aware of and adhere to HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency.
  • Resourcefulness, flexibility and integrity.
  • Ability to work independently.
  • Attention to detail.
  • Ability to function in a team-oriented environment.
  • Ability to make meaningful observations and document these findings.
  • Ability to organize and prioritize work within caseload to meet patient/family/payer needs and required documentation within required time lines.

 

 Essential Duties and Responsibilities:

  • Physician will treat patients from diverse populations.
  • Review patient files sent by other medical professionals.
  • Review medical history of the patients along with various lab reports and X-rays.
  • Regular case consultations with case managers, primary care providers, others as needed.
  • May request samples of blood, tissue or other bodily fluids from the patients for testing.
  • Identify disease and prescribe treatment plans and medication for the patients.
  • May have a case load of 10 – 20 patients per day. Follow up appointments as needed.
  • Monitors the occurrence of reportable and emerging diseases, analyzes infectious and chronic disease trends, and report findings to the clinic director for further evaluation of the Community.
  • May attend case review meetings, conferences and referral meetings.
  • Other duties may be assigned by the medical director or clinic director.

$200,000-$205,000.00
