Integrated Housing Specialist

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.

The Integrative Housing Specialist will be responsible for implementing and supporting the Integrative Housing Voucher Program.  The Housing Specialist will work collaboratively with staff, agencies and service providers to develop and provide stable, affordable, and long-term independent housing opportunities through short and long-term subsidies such as the Housing Choice Voucher program or other types of permanent housing. The Housing Specialist will provide assistance to adults with serious and persistent mental illnesses as well as offender history, and support the housing process from the institution they will be originating.  They will provide support with a recovery focus to patients; assists patients to discover and maintain a valued role(s) in the community, school and home.


The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how they can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities.  The individual must also posses the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

1.Is able to establish a rapport with patients.  Treats patients with dignity and respect and takes a patient centered approach to treatment, as noted through supervision, team participation, and feedback from patients and staff.  Is able to build a relationship of trust, as observed by patients, supervisor, and team.
2.Provides input into program issues and shares/receives feedback in a healthy, constructive atmosphere.
3.Has general knowledge about medications and diagnosis as well as a working knowledge of issues, rehabilitation and supportive living concepts associated with patients and uses these appropriately with patients.
4.Has working knowledge of crisis intervention techniques and counseling principles and remains current in EBP’s provided to patients.
5.With the assistance of the supervisor, is able to organize and prioritize a workload so that rehabilitative and administrative duties are completed on time an in accordance with productivity and quality standards.
6.Follows through with responsibilities to the satisfaction of contact persons and/or supervisor.
7.Learns the philosophy and operating procedures of Genesis and applies them effectively both within and outside the agency.
8.Represents the patients/program/agency constructively both within and outside the agency, serving on agency and community committees as appropriate.
9.Maintains general knowledge of community resources and presents these to patients/supervisor/team as appropriate.
10.Maintains a working knowledge of the benefits systems and utilizes this information to the best advantage of the patients.
11.Ability to use computers and word processing equipment.
12.Ability to express or exchange ideas by means of the spoken word and to receive detailed information through oral communication.  Primary functions include activities in which verbal communication with patients and with others is key to job effectiveness.
13.Visual acuity must meet a minimum standard to work with preparing, reading and analyzing data and reports, work with computer terminals, etc


Education:  Bachelor's Degree in Psychology, Social Work, Human Services or equivalent.

Experience:  1-2 years of experience with emphasis in mental health.

Licenses/certifications: Must possess valid driver's license.  Must maintain personal auto insurance at a level commensurate with agency requirements


Under the direction of the LTSS Coordinator, and within the center’s program policies and procedures, the Housing Specialist performs the following functions:

Essential Functions

Assists patients to maintain community living by teaching activities of daily living and personal care; promotes independent and productive habits to patients in the community. 
1.Assist individuals to secure and maintain affordable housing utilizing search activities such as contacting prospective landlords to schedule showings, scheduling housing interviews for individuals, assisting with the rental application process, and support individuals to make informed decisions. 
2.Provide housing assistance to ensure successful execution of a subsidy contract and to maintain current information about individual’s benefits to ensure that subsidy amounts are accurate with regard to individual’s income.
3.Provide housing advocacy and other support services to individuals.
4.Prepare reports which may include demographic information, services provided to individuals, and appointments with individuals, landlords, property managers, housing subsidy administrators, and marketing efforts.
5.Performs case management services for individuals as defined by the HeM’s in assisting individuals in gaining access to needed medical, social, educational and other services.
6.Completes case management assessment for clients and develops comprehensive care plans, may do individual treatment plans, and monitors services received.  Complete required documentation in a timely and thorough manner and according to agency standards.  Maintains standards of productivity as established for the position.
7.In collaboration with outside agencies, Prescriber, Emergency Services, families and natural supports, provides ongoing assessment, monitoring, support and crisis interventions to integrative clients as needed.
8.Participates actively in supervision, team meetings, client centered conferences and treatment planning for Integrative program individuals; makes contributions and applies what is learned effectively.
9.Monitors medications prescribed by prospective agencies to patients in the program as required and monitors compliance, provides education on prescribed medication and remains familiar with medications prescribed.
10.Establishes a staff development plan with supervisor and makes a concerted effort to seek training into areas identified in the plan.  Provides schedule, activities planned and outcomes and priorities within EMR and to supervisor.
11.May be required to transport patients in their personal vehicle.
12.May perform other related job duties as assigned by their supervisor.
13.Attendance is a requirement of this position.
