Our growing Partners in Health Program has a new Family Support Coordinator position in the Exeter, NH area.  

As the coordinator, you will provide family-centered family support services to families of children and youth with special healthcare needs. The coordinator will be assisting families in conducting needs assessments and helping with access to supports which may include financial assistance, services, inclusive community recreation, education, and special training. In conjunction with the Family Council, the coordinator will also organize projects and events in which families and youth can participate and create relationships with other families who have similar issues.

Associates Degree from an accredited program in a field of study related to health or social services is required, Bachelors preferred, along with one year of corresponding experience. The ideal candidate will have the ability to work independently and be able to complete paperwork timely and effectively. Experience with children with chronic healthcare needs and care coordination in health care or social service preferred. Previous experience with Office 365 and SharePoint strongly desired.  Candidates will be asked to engage in criminal background and motor vehicle record checks and have auto insurance which meets agency standards.
