Wellness Coach Team Leader

Summary Statement

The Wellness Coach Team Leader provides direct supervisory and administrative support to the InSHAPE Team in addition to leading community-based counseling services to adults with mental health issues to address emotional wellness through physical activity and balanced nutrition. The Wellness Coach Team Leader works collaboratively with clinical and medical providers to advance and support integrated care services for adults 18+ years old.  This is a full time, forty hour a week position.

Requirements:

Qualifications, Skills and Experience

The ideal candidate will possess exceptional interpersonal and relationship building skills; will have experience working collaboratively with one or more individuals to help accomplish a goal or behavior change; will be highly organized and have the ability to manage their own schedule with clients and group commitment; and have the ability to manage a balance of direct care and documentation responsibilities.

Additional desired experience includes training in motivational interviewing and motivational strategy building, being physically active, readily identifying with and believing in the holistic benefits of good health and possessing a commitment to decreased health disparity, and social stigma for individuals with mental illness.

Education

  • Bachelor’s Degree in Health Science, Social Work, Rehabilitation, Psychology, Education or related field;
  • Personal Trainer Certification and local gym membership are required and the responsibility of candidate to maintain during employment.

Physical Demands and Working Conditions

  • Ability to work in multiple settings (community, home, work, gym, etc.); regularly demonstrate or explain proper nutrition practices, physical fitness activities, techniques and procedures;
  • Ability to sit and stand for extended periods of time;
  • Ability to lift up to 25 pounds;
  • Manual dexterity to use computers and standard office equipment is required;
  • Reliable transportation, clean driving record and proof of adequate auto insurance is required as home and community visits to clients are expected.
Responsibilities:

Major Functional Areas and Description of Duties

Provides community-based support services to adults to insure implementation of client’s wellness goals; duties include the following:

  • Support the ongoing integration of community-based health and wellness programming within a community mental health care setting;
  • Supervise all program operations and functions to ensure competency of staff and proper data collection;
  • Promote whole health initiatives within the CMHC and integrate with Healthy Together;
  • Nurture and maintain community partnerships to supplement program resources and costs;
  • Follow evidence-based practice of the InSHAPE model to enhance client outcomes;
  • Engage Wellness Coaches, participants, and potential clientele in bi-annual celebrations;
  • Provide supportive and motivational counseling, client-centered goal planning, and fitness and nutrition training to improve mental, emotional, and physical wellness;
  • Execute treatment interventions that effectively address symptoms and behaviors that currently inhibit insight, independence, recovery and overall wellness;
  • Articulate and document treatment interventions effectively;
  • Create, distribute, monitor and administer wellness assessments;
  • Communicate symptoms, behaviors, strategies, and interventions to the clinical team;
  • Assist in instruction and facilitation for team building and group activities;
  • Work with staff to identify and establish resources and wellness support systems outside of treatment team/mental health agency;
  • Program develop and create new opportunities to improve the health and wellness of clients and staff;
  • Assist in training new Wellness Coaches and interns;
  • Maintain professional codes of conduct and ethics;
  • Perform any additional duties assigned by supervisor.
