Integrated Care Counselor

Established in 1920, Greater Nashua Mental Health has as its mission: empowering individuals to lead full and satisfying lives through treatment and support. Our agency provides comprehensive, evidence-based behavioral health services for individuals with mental health and/or substance misuse disorders. We offer a wide array of services in office and community settings, serving adults and children.  As the State-designated Community Mental Health Center for Southern Hillsborough County, we serve everyone who walks in our doors, and no one is ever turned away due to the inability to pay for services.  Without our agency, many of the most vulnerable members of our community would not have access to critically needed behavioral health care.   We believe that by respecting people as they are, we can help them to become all that they wish to be.

Job Summary:
Greater Nashua Mental Health is looking for a full-time Integrated Care Counselor to join our team! The Integrated Care Counselor’s primary purpose is to provide behavior change interventions within the integrated care department and the ProHealth wellness program. The Integrated Care Counselor provides consultation to identified partners regarding mental health concerns and wellness programming.

Duties & Responsibilities:
• Provide all behavior change interventions for the ProHealth wellness programs and research study.
• Meet regularly with partners in Integrated Care.  Consult regarding mental health needs in partner agencies and respond to requests for assessments.
• Review and develop treatment plans goals targeting integrated care and wellness interventions.
• Provide education and consultation to staff, other agencies and community groups about integrated care and ProHealth.
• Provide education to family/caregivers about ProHealth wellness programs.
• Assist Director of Integrated Care with marketing and promoting integrated health care services.

• Bachelor’s Degree or Master’s Degree in Clinical Mental Health Counseling, Social Work or related field. Preference given to Master’s Degree candidates.
• Minimum of 1-2 years of experience in a professional capacity involving the delivery of clinical services.
• Strong collaborating skills and an ability to work effectively both independently and as part of a team.
• Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
