Case Manager - Liberty House

Liberty House, a program of Catholic Charities New Hampshire, is currently seeking a dynamic individual to join our team as a full-time Case Manager. The schedule will be Monday through Thursday, 12PM to 8 PM, and Fridays, 8 AM to 4 PM, with some flexibility. The Case Manager is responsible assisting our resident and community Veterans in achieving self-sufficiency. The Case Manager will perform the functions of this position in accordance with current federal, state and local regulations as well as this agency's established policies and procedures.

Liberty House helps homeless and struggling Veterans by connecting them to available resources and providing a safe, substance-free transitional house. Our mission is to remember, honor and support our veterans.

Requirements:
  • Bachelor’s Degree in human services or related field (Master’s preferred)
  • Three years of experience working in human services or related field
  • Entry-level knowledge of addiction and issues specific to the Veteran homeless population such as Post Traumatic Stress Disorder (PTSD), traumatic brain injury, homelessness, and Social Model Theory
  • Attention to detail and ability to work independently and as a member of a team
  • Strong time management skills and ability to meet deadlines, and effectively manage competing priorities
  • Must assert sound judgment and maintain a high degree of confidentiality
  • Proficient in Microsoft Office Suite or related software
  • A valid state issued driver’s license required

 

Physical Demands:

  • The Case Manager is regularly required to talk and hear and walk or stand for brief periods of time
  • Employee is also required to exert up to 25 pounds of force occasionally or a negligible amount of force to lift, carry, push, pull or otherwise move objects
Responsibilities:
  • Connect with and develop relationships with member agencies and build and maintain relationships with our residents and community Veterans
  • Provide coaching opportunities to better equip our residents with the skills needed to succeed
  • Through advocacy and resource referrals, assist our residents with finding meaningful job placements and quality living arrangements
  • Build positive relationships with our residents and facilitate activities
  • Build positive relationships with volunteers, donors and other stakeholders
