Housing Stability Case Manager - Rapid Rehousing

Cross Roads House provides shelter, meals, basic needs, and supportive services to homeless individuals and families in the Greater Seacoast area. We provide both emergency shelter and a transitional shetler program to help our residents move with dignity and purpose to stable and decent housing. 


To promote sustained housing stability by providing community and home-based intensive case management services to families in a Rapid Re-Housing program. The Housing Stability Case Manager will also help program participants obtain safe and affordable rental housing, establish positive ongoing relationships with landlords, and work closely with referring agencies and community service providers. This position is grant-funded and part-time with flexible hours.

Requirements:

* Bachelor’s degree in social work or related field

* Demonstrated experience/skills in multi-tasking, priority-setting, working with people experiencing homelessness, collaboration, and crisis management

* Current drivers license, vehicle and insurance

* Eligibility for employment in the United States

* Ability to sit, stand, or walk for extended periods, indoors and outdoors

* A criminal background check will be conducted

Responsibilities:

* Work with partner agencies and other shelter staff to identify and determine eligibility of potential participants for the project.

* In cooperation with Program Director, complete homelessness and income eligibility and application materials with prospective participants and referring agencies

* In the course of collaborative work with participants, complete a Housing Budget, and assess for strengths and service needs, including benefits, employment, mental health, substance use, money management, social connection, and self-advocacy

* Assist participants in obtaining housing, including explaining program to and negotiating leases with landlords, completing apartment inspections, completing Participation Agreements with participants/landlords, and helping participants with apartment set-up and orientation to apartment living

* Support positive landlord-tenant relationships, as needed, and work to prevent lease violations and evictions

* Establish goals for a participant Housing Stability Plans, identifying appropriate steps towards achieving goals, modifying goals as needed, and tracking progress

* Provide home-based case management during and after the rental assistance phase of the program

* Facilitate a wide variety of community resource referrals, coordinate with service providers, and advocate on behalf of program participants as appropriate

* Utilize de-escalation, motivational interviewing, harm reduction, and other clinical skills to maintain participant engagement and address or prevent crisis situations

* Maintain precise and accurate documentation of case management services, including paper and electronic records

* Adhere to professional guidelines, including maintaining appropriate boundaries with clients and ensuring appropriate confidentiality

* Provide qualitative and quantitative information about participant progress, needs, and program outcomes, including using the HMIS data collection system as necessary

* Recommend and support improvements in program design and operations

* Participate in local and regional collaborative efforts and represent Cross Roads House as requested

* Other duties as directed
