Mental Health Court Case Manager
Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission. We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.
This is a great opportunity to provide supports with a recovery focus to adult patients that are court involved. The CCCM assists patients that are being released to the community and those already in the community in discovering and maintaining a valued role in the community. The CCCM interfaces with the agency, the court and the community to ensure optimum quality and comprehensiveness of services.
You’ll be part of a dedicated team of professionals that demonstrate our core values of respect, advocacy, integrity, stewardship, and excellence for our community, patients, and the agency’s health.
Bachelor’s degree in Human Services and 1-2 years of experience with emphasis in providing services to those with mental illness. Valid driver's license, vehicle and insurance required.
•Performs case management services as defined by the HeM’s in assisting individuals in gaining access to needed legal, medical, social, educational and other services.
•Completes case management assessment and develops comprehensive care plans and individual treatment plans and monitors services received.
•In collaboration with prescriber, emergency services, families, the court and natural supports, provides ongoing assessment, monitoring, support and crisis interventions to patients as needed.
•Works directly with the court and other legal personnel as indicated by the needs of the patient and may accompany the patient to court or attend court as indicated.
