Housing Community Case Manager

Lakes Region Mental Health Center

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.

Requirements:

Bachelor’s degree in mental health counseling, social work, psychology, nursing, counseling or other related field required. One to two years of experience with emphasis in mental health. Valid driver's license and vehicle required

Responsibilities:

•Maintain a caseload of 24 residents/patients.
•Assists individuals in gaining access to needed medical, social, educational and other services by assessment and developing of a comprehensive care plan, referrals and monitoring.
•Assists patients in applying for and/or maintaining financial benefits for which they are eligible. 
•Assists patients to maintain community living by modeling activities of daily living, budgeting, and personal care.
•Promotes independent and productive habits to patients in the residences by assisting patients in developing skills to attain needed services i.e. housing, medical appointments, food, etc. 
•Develops other programs and activities, which promote the introduction to and the integration of the patient into the community.
•Monitors medications prescribed to patients in the program as required
•Provides assessment, monitoring, support and crisis interventions to patients as needed.
•Manages a variety of tasks and demands.  Prioritizes competing needs in a way that is most helpful to patient and is an efficient use of time.
•Participates actively in supervision, client centered conferences, ISP planning, and staffing; makes contributions and applies what is learned effectively.
•Completes required documentation in a timely and thorough manner, according to established standards.
•Maintains standards of productivity as established for the position.
•Required to transport patients in their personal vehicle.
•Participates in agency committees and in other agency projects
•May perform other related job duties as assigned
