Emergency Services Community Case Manager
The Center for Life Management is a non-profit community mental health organization that has been serving the people and communities of Southern New Hampshire for 50 years.
As a provider of behavioral and mental health services, we are an organization that cares about and for people. We are dedicated to helping people achieve their optimal mental health and overall well-being, helping them maximize family and other important relationships in their lives, and providing supportive services to help them be as healthy, successful and productive as possible in their daily lives.
Summary Statement
Working within a community-based behavioral health setting, the Community Case Manager works within the Emergency Services Department to provide effective and quality services, coordinates and monitors other services provided, promotes independent functioning and develop self-sufficiency skills for CLM clients and their families. The comprehensive mobile crisis response team is a treatment model that covers for 24/7, 365 days of the year. The position is a five-day work week, forty hours per week.
Qualifications and Skills
Education and Experience
A Bachelor’s degree in social work or a similar discipline is preferred, however an Associate degree along with a minimum of two years of direct service mental health experience may qualify.
Physical Demands and Working Conditions
A majority of the work will be visiting clients in the community and also in their homes. Proof of car insurance, a reliable vehicle and a clean driving record all are required. An ability to freely enter/exit client’s homes is necessary.
