Case Manager, Community Support Services & First Episode Psychosis

Established in 1920, Greater Nashua Mental Health has as its mission: empowering individuals to lead full and satisfying lives through treatment and support. Our agency provides comprehensive, evidence-based behavioral health services for individuals with mental health and/or substance misuse disorders. We offer a wide array of services in office and community settings, serving adults and children.  As the State-designated Community Mental Health Center for Southern Hillsborough County, we serve everyone who walks in our doors, and no one is ever turned away due to the inability to pay for services.  Without our agency, many of the most vulnerable members of our community would not have access to critically needed behavioral health care.   We believe that by respecting people as they are, we can help them to become all that they wish to be.

We are seeking a full-time Case Manager to provide direct support, education, and outreach to a caseload of adults coping with mental health disorders. The primary focus is on meeting with individuals within their natural environments, coordinating community support systems and services, crisis intervention, education, and assisting in removing barriers to improving functions. The case manager will also be involved with a specialty team working with first episode psychosis (FEP) clients. The appropriate trainings will be provided and case manager will devote up to 50% of her or his time working in FEP. The Case Manager is a vital member of the multi-disciplinary team here at GNMH.

Candidate should have a Bachelor’s or Associate’s Degree in Psychology, Social Work or Human Services. Candidates with equivalent work experience will also be considered.  Previous experience working with people with mental disorders is preferred. Fluency in Spanish is a plus!
